Peruvaka Sree Muthappan Madappura Temple is dedicated to Muthappan, who is revered as the personification of two Hindu deities—Thiruvappan or Valiya Muthappan (Vishnu) and Vellatom or Cheriya Muthappan (Shiva).

The shrine where Muthappan is worshipped is known as Madappura. Devotees visit this temple to seek blessings from Muthappan, who is believed to protect them from evil and grant their wishes.
Festivals
The Temple has a number of festivals. The important ones are :
Vellattam
Vellattam is performed as an invocative ritual before the main Theyyam performance. This ritual is believed to transform the performer from a mortal human into a divine person, invoking the presence of Muthappan. Vellattam is a vital part of the temple's traditions, embodying the spiritual essence and power of the deity.
Vishu
Vishu is celebrated with traditional fervor here. The festival includes the Vishukkani, an auspicious arrangement of items viewed early in the morning to bring prosperity for the coming year. Devotees also partake in Vishu Kaineettam, where elders give money to younger family members, and enjoy the Vishu Sadhya, a grand feast featuring an array of traditional dishes.
