While Lord Shiva is the principal deity, the temple is most renowned for the worship of Sree Bhadrakali or Sree Parvathy, affectionately called Thirumandhamkunnilamma, along with Lord Ganesha.

Key rituals and offerings at the temple include Mangalya Puja, Rigveda Laksharchana, Chandattam, and Kalampattu.
Festivals
The Temple has a number of festivals. The important ones are :
Thirumandhamkunnu Pooram
Thirumandhamkunnu Pooram, the temple's grand annual festival, is an 11-day celebration and one of the most prominent festivals in Malappuram district. A highlight of the festivities is the Aarattu ritual, where the sacred idol of the Goddess is carried on the tallest caparisoned elephant, accompanied by five others, in a majestic procession to a nearby lake. There, the idol is ritually bathed by the chief priest before being reverently returned to the temple.
Rigveda Laksharchana
Rigveda Laksharchana is a sacred seven-day ritual conducted during the Mandala season, where esteemed Vedic scholars and Brahmins chant Rigveda mantras and offer flowers to the deity one lakh (100,000) times. This revered yagna, which began in 1977, has been performed annually without interruption from the 1st to the 7th of the Malayalam month of Dhanu. The ritual attracts a large gathering of scholars, learned Pandits, and devoted worshippers who come together to seek divine blessings.
