Many years back, Muthukadu, as the name suggests was a dense forest. Once an ascetic came to Muthukadu. He was having a mirror form of idol (Kannadi Bimbam) with him, which he used to do daily worships. God asked him to install the idol in the place and worship there. As a result he installed the idol there and started worshipping. Thus the place came to be known as Sree Muthukadu. The annual festival is conducted here to celebrate the divine presence of the deity in Muthukadu.
The Thalappoli Mahotsavam and Thiruvutsavam of Sree Muthukadu Mahadevan starts with Flag Hoisting Ceremony after Deeparadhana on Thiruvathira star of Makaram. In addition to the usual Tantric rituals, Thalappoll of Muthukattilamma is conducted. Thalappoli is a unique ritual where women and young girls carry thalam (plate) in their hands. Thalam is filled with rice, flowers and a lighted lamp. It is a symbol of prosperity. The next day afternoon famous Pooram of Muthukadu is conducted. For the Pakalpooram 9 elephants will be present. The elephants are decorated with gold plated Caparisons (Nettipattam), Bells (Mani), and Necklaces. Famous artists from across Kerala participate in the Chendamelam. On the fourth day of the festival, Cheriya Vilakku and Utsavabali is be held. Traditional performing arts, dance and music are staged during the festival. The utsava murti of the deity is taken out of the shrine for ritualistic bathing (Arattu) on the final day evening to Muppathdam Pathalam Kadavu.
